DVA Referrals for OT Adelaide
Enhance Occupational Therapists provide OT services to the Veteran community via the Rehabilitation Appliance Program (RAP).
Rehabilitation Appliance Program (RAP)
Occupational Therapy and allied health services are provided to DVA Health Card holders via the Rehabilitation Appliance Program (RAP). OT service may be short term to address specific needs or as part of the overall care plan.
DVA Health Cards (Gold and White) are accepted as payment upon a GP or hospital referral.

Referrals for DVA Health Card Holders
Letter of Referral
The Department of Veterans Affairs guidelines require that DVA health card holders have a letter of referral addressed to Occupational Therapist.
Standard Form (D0904)
The Department of Veterans Affairs supply a standard form that can be completed for referral of eligible DVA card holders.

Hospital referrals - Discharge Planning team can refer DVA health card holders to RAP.
Discharge Planning
When planning for hospital discharge, the medical team or clinical staff involved in discharge plans ( OT, physio, nurses) can refer DVA card holder to RAP providers.
